Seniors aged 65 and above are now eligible to apply for the CDCP online without needing an invitation letter. The application process is straightforward and can be completed on the official CDCP application form online.
What You Need to Apply
To complete your Canadian dental care plan application, provide the following information for yourself and your spouse or common-law partner (if applicable):
- Social Insurance Number (SIN)
- Date of Birth
- Full Name
- Home and Mailing Address
- List of any dental coverage you have through government social programs (if applicable)

After submitting your application, you can check its status through the following methods:
- Online: Use your Application Code or Client Number (found on the confirmation page of the online application or in the top right corner of your CDCP letter from Service Canada).
- By Phone: Call 1-833-537-4342 and select option number 2. You will need your Social Insurance Number (SIN) or other personal information on file to verify your identity.
